Psychosocial Supports

Our team work with our clients and their support circle to address the social, emotional and psychological barriers to fulfilling their goals.  This may include referral to a social worker, clinical psychologist or other mental health practitioner to address changes arising from the brain injury including secondary conditions such as depression, chronic pain and anxiety.

Our in-house social work services include:

  • Assessment to determine impact of complex psychosocial elements on a program
  • Counselling and practical assistance to reduce the impact of psychosocial issues
  • Counselling in adjustment to disability and grief and loss issues for client and significant others
  • Provision of support to informal support networks in their role as a carer and/or to manage the impact of a care program within their home
  • Practical assistance with managing changes to domains such as accommodation, financial matters, relationships, custody and access to children and liaison with government agencies such as Centrelink, NDIS and Community Services

The Access Team are experienced in the implementation of complex Positive Behaviour Support Plans. Positive Behaviour Support programs are a person-centred approach to deliver support for people who display challenging behaviours. They are a critical component of ensuring the person’s wellbeing as well that of their circle of support. Strategies are developed that help the person regain a sense of control over their everyday lives and experiences. Central to the approach is promoting positive relationships between the person and their caregivers.  We provide carer and family training to ensure that the plan is understood and that the outcomes are routinely evaluated to ensure they are meaningful and sustainable.
